Millions Still Lack Control Over HIV Despite Decades of Progress

Decades of progress against HIV/AIDS have not reached everyone. An analysis of 2025 data from 49 countries where the AIDS Healthcare Foundation (AHF) works found that roughly 33.9 million people live with HIV. More than 8.1 million of them do not have the virus under control.

The findings show that some countries have achieved remarkable results, while persistent gaps in diagnosis, treatment, and retention in care leave many people behind. AHF argues that governments must strengthen HIV services and make sure patients can stay on treatment without interruption.

Why Testing and Treatment Are Only the Beginning

AHF’s ongoing “Treatment = Life” campaign stresses that testing and starting therapy mark the first steps, not the finish line. Effective treatment lets people with HIV live long, healthy lives and reach viral suppression. That means the amount of HIV in the blood drops so low that standard tests cannot detect it.

People who achieve and maintain viral suppression cannot transmit HIV through sex. Advocates and health experts call this principle “Treatment as Prevention” (TasP). Reaching these outcomes takes reliable medicines, accessible services, and continued support that keeps people in care.

Antiretroviral therapy transformed HIV into a manageable chronic condition, and the global response has made enormous strides since. International targets known as 95-95-95 set the benchmark. They aim for 95% of people living with HIV to know their status, 95% of those diagnosed to receive treatment, and 95% of those on treatment to achieve viral suppression.

Progress remains uneven, though. Success at one stage of care does not guarantee success at the next.

Wide Gaps in Ghana and the Philippines

Ghana shows how people can slip away at each step. About 70% of people living with HIV in the country know their status, but only 56% receive treatment and 52% are virally suppressed.

The Philippines faces even wider challenges. Just 62% of people with HIV have a diagnosis, 41% receive treatment, and 40% are virally suppressed. In both countries, the numbers drop sharply as people move from testing toward long-term control of the virus.

These gaps underline the need to connect people to care quickly and keep them there. A diagnosis that does not lead to steady treatment leaves patients at risk and leaves the virus free to spread.

Scale Creates Challenges Even in Stronger Systems

Even countries with strong outcomes face difficulties because of the sheer size of their epidemics. South Africa reports 96% diagnosis coverage and 77% viral suppression. Even so, approximately 1.75 million people there remain without viral suppression.

By contrast, Lesotho has achieved 98% diagnosis coverage, 96% treatment coverage, and 95% viral suppression. That performance shows what sustained investment and effective HIV services can accomplish. It also gives other countries a concrete model for closing their own gaps.

Funding Pressures Add to the Concern

The disparities worry advocates all the more because funding pressures threaten the gains made so far. Services that diagnose, treat, and support patients depend on steady resources, and any disruption can break the chain of care. When people lose access to medicines, their viral load can rise again, which puts their health at risk and undermines prevention.

AHF’s message is that governments must treat HIV care as a continuous commitment rather than a one-time intervention. That means finding people who need testing, starting them on treatment quickly, and supporting them so they stay on it. With more than 8.1 million people still lacking viral control across the countries AHF analyzed, the work of the “Treatment = Life” campaign is far from finished.
